Job Description Summary
Facilitates coordinates and organizes behavioral health care in a team-based environment to meet patient needs and achieve optimal health outcomes for adult and pediatric patients. Provides care management services of mental or behavioral health conditions addresses social and psychosocial factors integrating behavioral health with medical care in support of the primary care practitioner.
Identifies and addresses social needs preventing optimal health. Connects patients with needed community resources and services to improve health outcomes.
Documents care and services in the electronic medical record as a member of the care team. Utilizes policies that support patients fundamental right to considerate care protects their personal dignity and respects their cultural psychosocial and spiritual values in an effort to meet patient care needs and preferences. Reports to the ambulatory care management leader. Performs other duties as assigned.
Responsibilities
Facilitates and coordinates behavioral health care in a team-based approach under the oversight of the primary health care practitioner to improve overall health of adult and pediatric patients
Performs initial assessment and administers validated screening tools documenting in the medical record
Develops the plan of care and provides systematic assessment and monitoring using validated clinical rating tools
Performs subsequent assessments revises care plans as needed in a continuous relationship with the care team
Educates patients about their condition and the treatment plan
Refers patients to behavioral health specialty care as needed
Receives referrals from practitioners within the medical group network
Uses telemedicine technology; may also provide in-person services
Communicates and collaborates with the primary practitioner and all members of the care team
Fields and arranges crisis call interventions as needed
Collects and reviews social determinants of health information to identify social or psychosocial issues that need to be addressed
Refers patients to community resources and services documenting in the medical record
Establishes relationships with community resources and tracks their success rates
Provides reports for the department and/or for the patient population served as needed including but not limited to social needs SDOH activity behavioral assessments and plans
Completes assigned goals participates in administrative meetings as needed
Performs other duties as assigned
Requirements -
Education
Preferred: Licensed Masters Social Work
Minimum: Licensed Bachelors Social Work
Experience
Minimum 2 years work experience in a medical/clinical setting preferred outpatient health care setting
Licensure Registration Certification
Minimum: LSW
