Position Overview
The Physical Therapist will provide skilled therapy services in the home setting supporting patients in improving mobility strength balance and overall function. This role requires strong clinical judgment and coordination with the care team.
Responsibilities
Perform in-home PT evaluations and reassessments
Develop and implement individualized treatment plans per physician orders
Provide skilled therapeutic interventions
Complete documentation accurately and in a timely manner
Educate patients and caregivers on home exercise programs and safety
Collaborate with physicians and interdisciplinary team members
Supervise Physical Therapist Assistants as applicable
Qualifications
Active Virginia Physical Therapist license
Minimum 1 year of experience (home health preferred)
OASIS experience preferred
Strong clinical and documentation skills
EMR experience preferred
Valid drivers license and reliable transportation
This Full-Time position serves patients in Blacksburg VA and surrounding areas.
