Human Touch Home Health is seeking an experienced Physical Therapist (PT) to provide skilled in-home therapy services in Warrenton and Culpeper Virginia.
We are currently hiring for Full-Time Part-Time or PRN positions based on your availability and preference.

What Youll Do
Perform comprehensive in-home PT evaluations and reassessments
Develop and implement individualized treatment plans per physician orders
Provide skilled therapy to improve mobility strength balance and function
Complete accurate and timely OASIS documentation
Educate patients and caregivers on home exercise programs and safety
Collaborate with physicians and interdisciplinary team members
Supervise Physical Therapist Assistants (PTAs) as applicable
What Were Looking For
Active Virginia Physical Therapist license
Minimum 12 years PT experience (home health preferred)
OASIS experience preferred
EMR experience (WellSky/Kinnser or Homecare Homebase a plus)
Strong clinical and documentation skills
Valid drivers license & reliable transportation
