Job Summary
The Advanced Practice Provider (Nurse Practitioner or Physician Assistant) functions as a primary healthcare provider within a collaborative care team. This role involves assessing diagnosing and managing patients across the lifespan including acute illnesses minor trauma stable chronic conditions preventive care and health maintenance.
The provider delivers comprehensive patient-centered care while adhering to established clinical policies and procedures and utilizing evidence-based practices.
Key Responsibilities

Obtain comprehensive health histories and perform physical psychological and developmental assessments

Assess and manage acute illnesses minor trauma pregnancy care (as applicable) and stable chronic conditions

Analyze and interpret diagnostic data to determine appropriate treatment plans
-
Initiate implement and prescribe treatment plans in collaboration with physicians as needed
-
Administer medications and injections within scope of practice
-
Order and interpret laboratory and screening tests
-
Conduct preventive screenings based on age and medical history
-
Document health status changes treatment plans and outcomes in electronic medical records
-
Provide patient and family education and promote health maintenance and wellness
-
Coordinate referrals follow-up appointments and use of community resources as appropriate

Serve as a resource for patients healthcare team members and community partners
-
Maintain accurate and timely documentation in automated systems
Qualifications
Education:
Graduate of an accredited Nurse Practitioner or Physician Assistant program
Experience:
None required
Licensure/Certification:
-
Current Licensed Nurse Practitioner (LNP) or Physician Assistant license
-
Registered Nurse (RN) licensure (if applicable to NP)
-
DEA license
-
AHA BLS-HCP certification required
-
Advanced Life Support certification preferred

Eligible for prescriptive authority in accordance with state regulations
Minimum Qualifications:

Ability to collaborate effectively with physicians and healthcare team members

Strong written and verbal communication skills

Proficiency with electronic medical record systems
