Job Summary
A Pulmonary Hypertension Patient Navigator provides case management services to patients at MLP and at MLP/ Sterns PHTN Multidisciplinary Clinic. This person is the main point of contact for PHTN patients. The Patient Navigator builds strong relationships with PHTN patients in order to help patients stay engaged in medical care and adhere to their medications. Patient Navigators are committed to removing barriers to care by identifying critical resources helping them navigate through health care services and systems and promoting patient health. They work closely with the patients care team assist in being a liaison between the patient and specialty pharmacy services as well as help find financial aid for medications to support positive outcomes.
Job Responsibilities
- Meet with Care Team to discuss patient care issues and needs and facilitate patient health care.

- Maintain documentation of all patient encounters and complete reporting requirements according to organization standards.

- Track patient information schedules files and forms in a confidential manner.
|
- Track patient attendance at medical appointments and patient navigation sessions and initiate outreach and missed appointment procedures as necessary.

Minimum Required: At least three (3) years healthcare experience.

Minimum Required: Graduate of an accredited Program in a healthcare related field.

Special Skills
Description
Minimum Required: Must be proficient in disease state and with all oral PAH therapies. Must also be knowledgeable about the approval initiation and maintenance of oral PAH therapies

Minimum Required: Holds a professional license in healthcare related field including but not limited to RN LPN RRT CRT.
