The Clinical Outcomes Coordinator is responsible for developing overseeing and evaluating clinical programs that impact population health outcomes for the medical center. This position will work closely with the Program Manager of Population Health. Analyzes best practices for intervention with specific conditions such as chronic illness social vulnerability or substance use disorder across the care continuum. Drives cross functional collaboration with clinicians finance and regulatory leadership to impact the population health strategic goals of the organization. Serves on workgroups both within the organization and externally with local state and industry groups. Functions as a clinical information systems consultant. Assists departments in designing documentation reports order sets and workflows related to data. Communicates data/outcomes to various audiences from front line to executive leadership. Performs special assignments as required and prepares detailed analytical reports for departmental and institutional use.

What you will bring:
Salary Range: Minimum /hour - Maximum /hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount salary discussions will occur during candidate screening calls before any subsequent compensation discussion is held between the candidate and any hiring authority.
