Job Summary
Empath Partners in Care a part of Empath Health is seeking a caring and results driven Medical Case Management Supervisor to join our team in St. Petersburg FL. In this role the Medical Case Management Supervisor will provide direct supervisory support to the senior case managers and medical case managers including but not limited to: hiring of new staff assignment of new clients training annual evaluations disciplinary actions and approval of time off.
What Youll Do
- Assists with the acquisition of public assistance Medicaid/Medicare ACA food stamps Social Security housing and other benefit programs.
- Maintain a small client case load not to exceed 30 clients.
- Works closely with the HIV Services Manager supports audits collects required statistics and contributes to policy and procedure development
- Coordinates transfer of clients from case manager to case manager when relocation is necessary. This may include transfers from other case management agencies.
- Gathers statistical information as directed/requested.
- Participates in internal and external audits.

What Youll Need
- Education and/or Experience: Bachelors degree (B.A.) from four-year college or university; or one to two years related experience and/or training; or equivalent combination of education and experience.
- Current drivers license automobile insurance and accessibility to a vehicle.
- Flexibility to attend meetings and trainings which may fall outside the regularly assigned work hours.
- Excellent communication skills and ability for detail.
