Includes OASISassessment at appropriate time written functional measurable goals treatments plans and home programs forclients. Identifies barriers to describe outcomes. Evaluates equipment needs to increasefunction level. Plans for treatment according to established goals on the treatment plan and reassesses theclients condition. Revises the plan of care in consideration with the physician and othercare-team members. Evaluate the effectiveness and outcome of clients/caregivers in appropriate activities and referrals to communityresources as appropriate utilizing both verbal and written evaluations and progress in the Clinical record and maintain client record asrequired by the agency with clients caregivers and all health care professionals required to providecomprehensive interdisciplinary Case Conferences at the Agency when physical therapy clients are skills and knowledge though training and continuing education to provide expectedstandards of care.
