Job Details
Interprets physicians prescriptions and dispenses medications for high dollar specialty and other medications within regulatory accreditation and policy guidelines. Provides medication therapy management by actively managing drug therapy; identifies prevents and resolves medication-related problems. (e.g. Counsels patients and/or medical staff and handles problem resolution). Completes patient records initial assessments medication profiles initial fill notes and continuing progress notes. Participates in the coordination of care to meet patient outcomes and goals including initial referral medication review and selection assessment of medication delivery system coordinated care conferences patient care plans compliance programs and discharge of patients from services.Job Description

OTHER FUNCTIONS AND COMPETENCIES needed in addition to Essential Functions:
Documents any and all interactions with patients and prescribers in the appropriate systems in compliance with protocol.
Performs On-Call activities as required.
Adheres to all pharmacy state and federal laws and regulations.
Performs other duties as assigned.
POSITIONS SUPERVISED OR DIRECTED:
Specialty Pharmacy Representatives I and II Rotation IPPE and APPE students
EDUCATIONAL/TRAINING REQUIREMENTS:
PharmD degree with at least 3 years of Pharmacy experience or 1-2 years of Specialty Pharmacy experience is preferred.
CERTIFICATES LICENSES AND REGISTRATION:
Valid Pharmacist licensure in the State of Pennsylvania
EXPERIENCE REQUIREMENTS:
Prior specialty pharmacy experience or residency preferred
