Essential Functions

• Interacts with co-workers visitors and other staff consistent with the values of Jefferson.

• Assures the appropriateness of drug therapy and compliance with hospital policies

• Oversees the distribution of medications throughout TJUH

• Supports implementation of processes to assure the quality of Pharmacy services

• Trains pharmacy technicians residents students interns and new pharmacists as assigned.

• Precepts student pharmacists AND residents as assigned.

Competencies (Knowledge Skills and Abilities Required):

• Knowledge of and ability to apply pharmacokinetic principles for monitoring and dosing medications

• Monitor patient response to medications and provide recommendations on management if appropriate

• Identify and document sentinel/serious events near misses medication errors and adverse drug reactions in the electronic event reporting system

• Provide quality patient care in accordance with standards of practice demonstrating high level expertise competency knowledge and skills

• Promote and assure compliance with the hospital formulary and policies and procedures defined by the P & T Committee.

• Provide evidence-based recommendations to support patient care

Education: Doctor of Pharmacy AND Experience:

• Completion of PGY2 Pharmacy Specialty Residency; OR Completion of PGY1 Pharmacy Residency PLUS 2 years of relevant experience; OR Completion of no residency PLUS 5 years of relevant experience

Minimum Certifications Registration or License Requirements:

• Pharmacist License in the State of Pennsylvania
