Position Summary:
The Patient Flow Managers role is to coordinate the overall patient flow process effectively minimising delays and supporting discharge. He/she will work with all departments maximising patient throughput and utilisation of available hospital beds and also ensuring all beds available to the hospital in other facilities (off site) are utilised in the most efficient and effective manner. The Patient flow manager will be responsible for the coordination of all patients discharge including complex discharges using pathways such as the Long term care process.
Key Tasks:
1. To support egress and flow of patients that are admitted in the emergency department directly to the ward.
2. To maintain an accurate and timely knowledge of bed availability using the Visual Hospital lean methodology. To ensure real-time patient discharge is recorded and beds assigned for patient care in accordance with hospital policies.
3. To manage the split of urgent/emergency and scheduled care to effect the daily placement of unscheduled and scheduled admissions efficiently.
4. To work with appropriate members of Interdisciplinary teams inclusive but not exhaustive of medical health & social care professionals along with nursing staff to ensure safe admission and discharge of all patients.
5. Proactive case management of all patients discharge in accordance with the complex discharge policy including LTC process and HSS process management to ensure efficient flow throughout the hospital.
6. Liaise closely with community medical teams nursing & hospital management in relation to performance indicators such as core occupancy delayed discharges length of stay analysis.
7. To collaborate with management information systems in collating data on the frequency of peaks and troughs of bed usage order that the hospital management can be better informed and work towards more effective bed utilisation planning of theatre lists and other services demands.
8. Perform such other duties appropriate to the office that may be required from time to time.

Selection Criteria:
Selection criteria outline the qualifications skills knowledge and/or experience that the successful candidate would need to demonstrate for successful discharge of the responsibilities of the post.
Applications will be assessed on the basis of how well candidates satisfy these criteria.
Mandatory:
1. Statutory Registration Professional Qualifications Experience etc
(a) Eligible applicants will be those who on the closing date for the competition:
(i) Are registered in the relevant division of the Register of Nurses & Midwives
maintained by the Nursing and Midwifery Board of Ireland NMBI (Bord
Altranais agus Cnáimhseachais na hÉireann) or entitled to be so registered.
(ii) Have at least 5 years post registration experience (or an aggregrate of 5 years
fulltime post registration experience) of which 2 years must be in the speciality
or related area.
(iii) Have the clinical managerial and administrative capacity to properly discharge
the functions of the role.
(iv) Candidates must demonstrate evidence of continuous professional development.
(b) Candidates must possess the requisite knowledge and ability including a high standard of
suitability and clinical managerial and administrative capacity to properly discharge the
functions of the role.
Hold a Level 9 qualification on the National Framework of Qualifications.
Hold a postgraduate course in Oncology/Haematology.
2. Annual registration
(i) On appointment practitioners must maintain live annual registration on the relevant
division of the Register of Nurses and Midwives maintained by the Nursing and
Midwifery Board of Ireland (Bord Altranais agus Cnáimhseachais na hÉireann).
(ii) Confirm annual registration with NMBI to the HSE by way of the annual Patient
Safety Assurance Certificate (PSAC).
3. Health
Candidates for and any person holding the office must be fully competent and capable of
undertaking the duties attached to the office and be in a state of health such as would indicate a
reasonable prospect of ability to render regular and efficient service.
4. Character
Candidates for and any person holding the office must be of good character.
Desirable
1. A management course is desirable.
2. Relevant experience as CNM1 is desirable.
If an appointee has previously been employed in the Civil or Public Service and that appointee is entitled to or in receipt of a pension from the Civil or Public Service or where a Civil/ Public Service pension comes into payment during the appointees re-employment that pension will be subject to abatement in accordance with Section 52 of the Public Service Pensions (Single Scheme and other Provisions) Act 2012.
