As a Post Secondary Transition Counselor at The Pinnacle School you will prepare students for successful transitions into adulthood by combining counseling program development and family collaboration. Youll empower students to build independence life skills and confidence while guiding staff and families through each phase of transition planning.
What Youll Need
Masters degree in counseling psychology social work or a related field
Valid Connecticut endorsement: 068 (School Counselor) 070 (School Psychologist) or 071 (School Social Worker)
Strong interpersonal communication and organizational skills
Experience supporting students with learning or emotional challenges
Ability to collaborate effectively with families districts and multidisciplinary teams
What Youll Do
Apply therapeutic frameworks (CPS CBT DBT and family systems) to support emotional regulation and intervention planning
Develop individualized transition plans life skills programs and community-based internship partnerships
Collaborate with educators clinical teams and families to ensure continuity between school and postsecondary goals
Lead multidisciplinary meetings to align academic behavioral and transition objectives
Use data and professional feedback to evaluate student progress and improve program outcomes
