Job Summary
Empath Home Health a member of Empath Health is seeking a caring and motivated PRN Home Health Occupational Therapist (OT). The Home Health Occupational Therapist provides exceptional care to our clients in Lee County (Fort Myers) area.

What youll Do
- Provides treatment to the patient in compliance with agency policies and procedures and federal and state regulations.
- Performs initial and on going assessments to determine level of functioning and patients ability to perform activities of daily living including OASIS assessments at appropriate time points.
- Develops and revises a plan of care in consultation with the physician and other care team members which will enable patient to achieve realistic goals of self-care.

What Youll Need
- Bachelor Degree in Occupational Therapy.
- Florida licensure in occupational therapy.
- Minimum of one (1) year experience as a licensed Occupational Therapist.
- Home health care experience preferred.
