Average Daily Hospital Practice Census: 10-12/day with APPs to help round

Facility Reimbursement:
Flights - $750/round trip cap (to include all taxes and fees). No upgraded airfares or seat upgrades
Lodging - 1.5x the GSA rate cap average for the total stay (to include taxes and fees). Preferred Hotel: Staybridge Suites
Rental Car - $55/day cap (including all taxes and fees) regardless of car class. SUV/upgrades over cap will need to be preapproved (will only be approved if needed for inclement weather). Preferred Rental Car: Enterprise and National
Mileage - IRS Standard Rate
Bill Rates Needed for Submission:
Malpractice (must be included in the rates)
Orientation
24 Hour On Call ($9000 rate cap to include 14 gratis hours)
Holiday 24 Hour On Call ($13500 to include 14 gratis hours)
Call Back ($590 per hour after 14 gratis hours)
Holiday Call Back ($885 rate cap)
Conversion fee ($25k rate cap)

Requirement description :
Board Certified in Gastroenterology or Truly Board Eligible in Gastroenterology (within 5 years of completing a Gastroenterology Fellowship) - REQUIRED
Active IA License or Active IMLC (please upload LOQ at time of name clear) - REQUIRED
Must be Fellowship Trained in Gastroenterology - REQUIRED
Must be experienced - REQUIRED
Must be able to do EUS & ERCP - please note at time of name clear - REQUIRED
Must be able to cover these dates: June 12 @ 0700 - June 19 @ 0700 - please note at time of name clear - REQUIRED
Clean Malpractice/Background History - REQUIRED - please upload NPDB (must be pulled within the last month or within 90 days)
