Position: Full-Time
Hourly Rate: $16.25 - $19.66 (3 levels based on experience and education)
FLSA Status: Hourly/Non-Exempt
Reports To: Clinical Manager and/or Clinical Director
Department: Outpatient and WrapAround Services
Location: Anthony New Mexico
Job Summary
The Intake Specialist is the first person to greet and navigate the needs of people seeking services
at FYI . The resource specialist is responsible for harmonizing multiple systems of response and services
through referral systems service coordination screening and triage. Intake Specialist will also engage
in outreach efforts and develop community relationship in efforts to provide consumers with a wide selection
of services to meet the presenting needs. This role is intended to be a multi-service position that will require
the ability to ensure customer service customer satisfaction and service delivery from a trauma informed/
responsive lenses
What Youll Do:
- Provide confidential administrative support related to sensitive issues
- Triage customers needs and guide them to the appropriate pathway of services
- Assists customers in completing all on-boarding paperwork and filling out applications and other forms
- necessary to access other health care professionals regional agencies and community service programs.
- Coordinate warm hand offs between providers for each pathway to services determined.
- Serve as a community referral link for customers to internal and/or external services and service providers.
- Schedules new patients for intake assessments with therapists daily
- Monitors scheduled intakes daily if there is a no show or cancelation navigation specialist will schedule
another patient
- Manage agency data tracking systems for all referrals that flow through the Resource and Referral
- Manage NM Star database and record all required data relevant to Treat First
- Verify Private insurance as well as Medicaid eligibility (via NM Medicaid portal) for clients
What You Need:
- Bachelors Degree in a human service field from an accredited university and one (1) year relevant
experience working with the target population; or hold an associates degree in a human service field from an accredited college and have a minimum of two (2) years of experience working with the target
- Knowledge of psychiatric and mental health conditions
- Strong interpersonal and communication abilities
- Familiarity with community resources and referral networks
Preferred:
- Bilingual (English/Spanish)
- Experience working with both adults and youth
