This position is responsible for the development and implementation of DaVitas Patient Pathways Programthrough facilitating patient education processing of timely dialysis placement referrals and buildingrelationships. This position enhances relationships with local hospitals doctors and other kidney care providersby enabling patients to make informed decisions regarding their dialysis provider. This position is responsible for
improving the patient education process by analyzing and sharing best practices with other members of thedepartment.
ESSENTIAL DUTIES & RESPONSIBILITIES

ESSENTIAL FUNCTION
1. Patient Education: Communicate with and treat patients withempathy at all times. Meet individually with patients requiringoutpatient dialysis placement depending on program scope withinthe assigned region to provide educational materials on: Outpatientdialysis Modality options Vascular Access options insurance whichincludes an assessment of patients insurance out-of-pocket costs and grant program options. Provide education on options forpatient to select an out-patient dialysis center of their choosing. Identify resolve or escalate patient issues and/or barriers toplacement to appropriate teammates for prompt and effectiveresolution.
2. Outpatient Dialysis Placement Referral: Work with hospital CaseManagers Social Workers and Physicians to place patients inoutpatient dialysis clinics. Arrange out-patient kidney dialysisplacement for patients by contacting local clinics within anappropriate proximity as determined by patient preference and
providing all required medical documentation to the outpatientdialysis center. Assist with patient transitions from hospitals todialysis facilities including transfer of paperwork and as much aspossible ensure patient satisfaction. Place patients and documentreferral in accordance with Patient Pathways Policy and Procedures.
3. Relationship Building: Work closely with both internal and external
kidney care professionals to enhance current relationships ordevelop new relationships. Partner with Regional OperationsDirectors and Facility Administrators to identify and implement bestpractices for accepting patients from inpatient acute care settingsinto an outpatient dialysis provider. Attend operational teammeetings with multi-affiliate providers in assigned region.
4. Reporting: The above statements describe the general nature and level of work being performed by individuals employed in this position. This job

constitute an employment contract. 2/3 reports at a minimum of one time per month. Record patient datain electronic tracking system according to Patient Pathways Policyand Procedures. Immediately inform Regional Operations
Manager/Supervisor of any reporting requests from the Regional Operations Manager/Supervisor in gathering patientdetails as needed for reporting purposes. Attend hospital meetingsto review reporting as needed. Adhere to all audit compliance andreporting requirements according to Patient Pathways Policy andProcedures.
5. Innovation: Learn develop and implement new ways for Patient
Pathways to differentiate itself in the marketplace. Ensure Patient
Pathways leadership is apprised of customer needs and anydeviations from current practices. Participate in special projects asneeded or requested. Educate team local markets on PatientPathways program and initiatives.
6. Other duties as assigned.
