Job Description
Your Impact:
- Deliver Field-Based Care:Conduct comprehensive primary care visits and physical exams in the comfort of a patients home.
- Manage Chronic Disease:Create longitudinal care plans that move the needle for high-risk patients.
- Collaborate Across the Continuum:Partner with an interdisciplinary team to ensure seamless safe care transitions.
- Prevent ER Visits:Treat acute needs in the home to keep seniors safe healthy and out of the hospital.
Job Requirements
What You Bring:
- Advanced Degree:Masters Degree (NP) with a preferred focus on Adult Geriatric and/or Family.
- Illinois Credentials:Active unencumbered NP license and Board Certification (ANCC or AANP).
- Reliable Mobility:Valid drivers license and vehicle for local field-based travel.
