As an LTC RN you will assess residents implement and adjust individualized care plans administer medications monitor vital signs manage wound and skin care and collaborate with physicians therapists and dietitians to optimize health outcomes. Youll lead fallrisk prevention strategies coordinate transitions of care and serve as a critical link between residents families and the care team. Documentation in electronic medical records accurate charting and adherence to regulatory standards will be part of your daily routine with a focus on accuracy empathy and timely communication.
