Social Worker, Homecare-Hospice MSW

Job Summary
Homecare/ Hospice:
- Provides assessment and supportive services to help meet the patient/family social emotional economic and environment needs in relation to the impact of terminal illness though education support/counseling and referrals/coordination of community resources.
- Provides guidance education and emotional support to patients/families at the end of life.
- Completes timely visits and accurate documentation to meet patient/family needs and ensure program compliance.
- Collaborates with the interdisciplinary team to provide whole person care addressing bio-psycho-social needs of patients and family.
Provide patients/families with the psychosocial support needed to cope with terminal illness and end of life through education support/counseling and referrals for community resources.
Work Schedule: 20 hrs. weekly / 0.5 FTE; Days
Required Qualifications:
- Masters degree in social work from a CSWE accredited program
- Must have Minnesota (MN) or Wisconsin (WI) Social Work license or license eligible upon start date:
- MN must have one of the following:
- Licensed Graduate Social Worker (LGSW) or higher licensure.
- If LGSW must be actively working on obtaining MN licensure at the LISW or LICSW level.
WI must have one of the following:
Certified Advanced Practice Social Worker (CAPSW) or higher license level.
- MN must have one of the following:
If licensed in only one state upon hire must be willing to obtain license in the other state and provide proof of license within 4 months of start.
Must have vehicle drivers license and current auto insurance. Must meet all criteria required under Lakeviews Automobile Liability Guidelines
Preferred Qualifications:
- Masters degree in Social Work
- Licensure/ Registration/ Certification: Must have Minnesota (MN) or Wisconsin (WI) Social Work license or license eligible upon start date:
- The following MN license is preferred:
- Licensed Independent Social Worker (LISW) or
- Licensed Independent Clinical Social Worker
- WI must have one of the following:
Advanced Practice Social Worker or
Licensed Clinical Social Worker
- The following MN license is preferred:
