In this role you will assess patients develop and implement individualized treatment plans monitor progress and document outcomes in electronic health records with precision and care. Your caseload may include musculoskeletal injuries post-surgical rehabilitation geriatrics services and pelvic health considerations all while guiding patients through therapeutic exercises manual therapy gait training balance work and activity modification. Youll lead interprofessional collaboration with physicians nurses occupational therapists and other physical therapists to coordinate care optimize mobility reduce pain and restore independence. Education and communication are core componentsempowering patients and families with clear explanations of home exercise programs prognosis and realistic milestones.
