Job Summary
About the role
As a Support Coordinator you will work closely with participants to navigate the NDIS and interpret their individual packages; it is your role to ensure they are getting the most out of their NDIS plans.
The key responsibilities of a Support Coordinator include but are not limited to:
- supporting and empowering participants to make their own choices as to how they want to lead their lives
- coordinating a variety of services for participants to support them to live fulfilling and meaningful lives within the community
- engage with and managing participants key stakeholders (including government agencies community and clinical service providers and families) you will ensure that the supports and services are leveraged not duplicated
- working with participants to strengthen their ability to coordinate their own supports to live their best life
About you
To be successful you must:
possess a working knowledge of the NDIS (is desirable)
excellent communication skills
a commitment to best practice
experience working in a team environment
the ability to quickly and effectively engage consumers
the ability to work with culturally and linguistically diverse communities & individuals
Role Requirements
- certificate IV in Mental Health (desirable)
- a current minimum P1 C class Queensland drivers licence
- a currently registered roadworthy and fully comprehensive insured vehicle
- first Aid and CPR certificates (ability to obtain)
- a Positive Notice Working with Children (blue card) (ability to obtain)
- NDIS Worker Screening Clearance (ability to obtain)
- NDIS Worker Orientation Module Quality Safety & You
