- Provide skilled nursing services to patients in their homes in accordance with physician orders and regulatory guidelines.
- Conduct patient assessments monitor health status and document care provided.
- Develop implement and update individualized care plans based on patient needs.
- Educate patients and their families on health management medication administration and prevention strategies.
- Collaborate with healthcare providers and interdisciplinary teams to ensure coordinated patient care.
- Maintain accurate and timely documentation in compliance with legal and organizational standards.
Requirements
- Current Registered Nurse (RN) license in the state of Michigan.
- Previous home care or community health nursing experience preferred.
- Strong interpersonal and communication skills.
- Ability to work independently and manage time effectively.
- Valid drivers license and reliable transportation.
- CPR certification required.
