Youll partner with physicians case management colleagues social workers and ancillary teams to manage risk ensure payer compliance and optimize outcomes. Your expertise will drive discharge planning post-acute follow-up and the removal of barriers to carewhether that means arranging transportation securing durable medical equipment or coordinating multidisciplinary rounds. Youll educate patients and families advocate for culturally competent care and use data to shape improvements in pathways and policies.
