Summary
Responsible for managing all aspects of the Shared Living Program including but not limited to promoting the program identifying individuals and providers negotiating agreements with and training providers supporting individuals managing emergencies monitoring and evaluating providers and acting as a liaison to the funding source.
Hours: 40 hours Monday- Friday : 9am- 5pm. This job is flexible and candidates should be available to respond to emergency situation on weekend if needed in rare occurrences.
Office: Chelmsford
Local Travel Required Milage reimbursement.
Essential Job Functions
  • Identify and source Provider candidates determine appropriateness and evaluate whether candidates meet minimum criteria.
  • Become familiar with providers values interests likes dislikes and family composition to facilitate a match with an individual in their home.
  • Negotiate provider contracts annually; approve provider use of respite care; monitor provider use of respite.
  • Maintain provider records which include but are not limited to evidence of provider certification and re-certification compliance with training and other requirements evidence of routine visits made by the shared living coordinator.
  • Routinely (at least once a month) visit providers; assure that problems that may impact the success of the placement are addressed.
  • Connect the provider to resources and assist the provider in building natural community supports.
  • Determine the training individual and groups of providers need; develop and/or make arrangements to purchase training for providers; ensure providers receive training and on-going supports necessary to successfully support an individual in their home.
  • Plan and host quarterly forum for providers.
  • Ensure providers comply with all conditions in their contractual agreement with LifeLinks.
  • Conduct intake assessments of people referred for Shared Living by DSS and other non-Incompass referral sources; determine appropriateness for Shared Living model and Incompass ability to find a matched provider.
  • Work with individuals identified for Shared Living Placement in collaboration with senior group home management; work collaboratively to plan transition from the group home to the shared living system.
  • Develop a sound knowledge of the individuals history strengths and needs.
  • Collect key information about the individuals likes interests and needs which will facilitate making a match with a provider.
  • Match individuals with providers based on a thorough assessment and evaluation process.
  • Prepare Individual Support Plan paperwork for each individual on caseload to include completion of assessments establishment of goals and strategies to achieve goals.
  • Support the individual to live successfully with the provider by visiting the individual routinely and by providing direct care support to the individual to better understand the individuals needs and provide observational data to the provider.
  • Utilize consultants as needed (behavior nursing OT PT etc.) to assess individuals changing and/or unmet needs and provide suggestions on how to support the individual better.
  • Visit the individuals day habilitation service periodically; monitor regular and satisfactory contact between individual and provider.
  • Establish and maintain accurate updated clinical records for each individual served.
  • Ensure the family of individuals and the individuals satisfaction with service; resolve disputes and/or address concerns.
  • Manage a caseload of shared Living matches.
  • Help resolve transportation problems.
  • Complete safety assessment in the providers home with the individual present. Unsure that the Safety Plan is written; secure signature sign-offs and approval from DDS ensure plan remains valid and is updated.
  • Ensure the provider and the individual have time apart by planning vacation respite and other opportunities.
  • Ensure incident/accident reports are completed as required.
  • Work collaboratively with Investigators on allegations of abuse (if not in conflict with mandated reporter role).
  • Act as a liaison to DSS; meet routinely; ensure satisfaction with service; resolve problems efficiently and effectively.
  • Work with Assistant Director to ensure expenditure are within shared living services budget.
Qualifications

Education and Experience
  • Bachelors degree in social work psychology sociology human services or other social service area
  • Five years of experience in direct support working with persons who have developmental disabilities.
  • Consideration will be given to candidates with an equivalent combination of education and related work experience
  • Ability to work independently
  • Knowledge of DDS regulations systems and the service planning process.
  • Position requires experience working with families and/or mentoring.
Computer Skills
Demonstrated proficiency in use of e-mail the Internet and have basic Word and Excel skills.
Analytical Skills
Demonstrated ability to problem-solve handle emergencies and successfully manage several projects simultaneously.
Language Skills
Mastery of the English language required as demonstrated by the ability to read and write instructions ability to read analyze and interpret field and/or professional journals technical procedures policy and procedure manuals and governmental regulations. Exceptional interpersonal skills and the ability to respond to common inquiries or complaints from Individuals families staff or the general public. Demonstrated ability to comprehend and follow verbal/written instructions. Ability to write reports business correspondence and procedures. Ability to effectively facilitate meetings.
Mathematical Skills
Ability to add subtract multiply and divide all units of measure using whole numbers common fractions and decimals. Ability to compute rate ratio and percent and to interpret bar graphs.
Reasoning Ability
Ability to apply common sense understanding to interpret and carry out a variety of instructions furnished in written oral diagram or schedule form. Ability to solve practical problems and deal with a variety of abstract and concrete variables in situations where standardization exists.
Certificates Licenses Registrations
  • Valid Drivers License
  • Current First Aid and adult CPR certification required within first 3 months on the job; thereafter employee is expected to remain current with First Aid and adult CPR certification.
Physical Demands

Specific vision abilities required by this job include close vision distance vision peripheral vision and the ability to adjust focus.
