Job Summary
University Home Care Inc is seeking a compassionate and dedicated Registered Home Care Nurse to provide high-quality nursing care to patients in Farmington Hills and the surrounding areas. This role involves delivering personalized in-home healthcare services to support the well-being and independence of our clients.
Responsibilities
- Provide skilled nursing care to patients in their homes including assessments treatments and medication management.
- Develop and implement individualized care plans based on patient needs and physician orders.
- Monitor patient progress document observations and communicate updates to the healthcare team and family members.
- Educate patients and their families about health conditions care techniques and prevention strategies.
- Coordinate care with other healthcare professionals to ensure comprehensive and continuous care.
- Adhere to all regulatory and organizational policies and procedures.
Requirements
- Current Registered Nurse (RN) license in the state of Michigan.
- Previous experience in home care or community nursing preferred.
- Excellent interpersonal and communication skills.
- Ability to work independently and manage a caseload effectively.
- Valid drivers license and reliable transportation.
