An opportunity has arisen to join the Digital Care Team as a Senior Independence and Wellbeing Practitioner Digital Discharge Coordinator working on-site at Ipswich Hospital.

In this role you will play a key part in supporting patients families and staff to use the Cassius care technology service particularly within hospital discharge pathways. Youll work closely with social workers fellow Independence and Wellbeing Practitioners and a wide range of health and community partners to ensure the best outcomes for individuals.

This is an exciting chance to gain hands-on experience in delivering care technology driving change and improving peoples independence and wellbeing. The work is both rewarding and fast paced.

Key responsibilities

  • Champion the use of care technology within discharge planning and support professionals to incorporate it effectively.
  • Carry out and support care technology assessments.
  • Work with community teams and INT Coordinators to manage referrals aimed at preventing hospital admissions.
  • Manage the full referral process including recording tracking and monitoring benefits and outcomes.
  • Support communication and awareness raising of the Cassius service across teams.
  • Make informed decisions regarding eligibility for care technology solutions.
  • Assist the Alcove Team in prioritising installations.
  • Deliver training to therapy teams across hospital and community settings.
  • Develop and shape the role in response to changing business needs.
  • Work as part of an integrated team across the NHS Social Care and Cassius partners.

We are looking for someone with

  • Excellent communication skills.
  • The enthusiasm to contribute to a multi-agency approach for responding within urgent timescales.
  • Resilient under pressure and the ability to remain positive when challenged.
  • Ability to adapt to changes within the role as and when required.
  • Excellent knowledge of care technology and Cassius and how this can positively improve quality of life and maintaining adults independence.
  • Previous experience of working within hospital discharge pathways.
