Job Summary

Job Title: Registered Nurse Case Manager Hospice
Location: Mount Prospect IL & Surrounding Areas
Schedule: Monday Friday 8:00am 5:00pm (hours may vary based on patient needs)
Compensation: $80000 $85000 annually $10000 First-Year Success Bonus

A healthcare organization is seeking a compassionate and experienced Registered Nurse Case Manager to provide hospice care in Mount Prospect and surrounding communities. This role offers the opportunity to lead patient care coordinate with healthcare teams and make a meaningful impact on patients and families while working within a supportive clinical environment.

Key Responsibilities:

Manage and coordinate patient care in hospice settings
Develop and oversee individualized care plans
Provide leadership guidance and support to nursing staff
Collaborate with interdisciplinary healthcare teams to ensure quality care
Deliver compassionate support to patients and families
Maintain accurate and timely clinical documentation

Required Qualifications:

High School Diploma or GED (Associate degree or higher preferred)
Active Illinois RN license
Minimum one (1) year hospice experience
Minimum one (1) year management or leadership experience
At least one (1) year of experience in home health care or Epic Home Health
Current AHA BLS and ACLS certifications
Willingness and ability to travel locally (territory covers approximately 25 30 mile radius)
Strong leadership clinical assessment and communication skills

Schedule & On-Call Requirements:

Monday Friday standard hours 8:00am 5:00pm
One (1) 24-hour weekend shift per month (8:00am 5:00pm field visits on-call 5:00pm 8:00am)
Two (2) weekday on-call shifts per month (5:00pm 8:00am)
Additional stipends provided for on-call coverage
Two holidays per year (one summer one winter) plus three floating holidays
Dedicated after-hours RN coverage available
