About the Role

As a Community Support Worker you will meet with individuals families and carers to connect and listen and where necessary undertake an assessment under the Care Act 2014. Where there is an eligible need you will devise a plan for how the person can access the support they require this may be connecting with their local community referral on to other agencies or provide service to support their unmet need. Where necessary you will identify and report potential safeguarding consider mental capacity and other high-risk situations where a persons safety or liberty may be at risk. Your team will have an office base and can work in a hybrid way. You will undertake visits to people in their own homes in a hospital setting or wherever the person feels most comfortable to meet you.

The Cognitive and Physical (C&PD) Team provides support for adults with physical disabilities and older people with eligible needs related to frailty and cognitive disabilities arising from long term conditions such as Dementia Stroke Parkinsons Disease and Brain Injury. The service will support adults with early onset dementia including alcohol related dementia where the primary need is linked to needs arising from dementia rather than alcohol dependency. Longer- term and more complex work will be done in the C&PD teams with short term interventions being the responsibility of the Home First service.

The focus of C&PD work is to promote independence and opportunities with a focus on outcomes that promote wellbeing.

Locality based C&PD teams include Community Support Worker Social Worker and AHMP posts and will be responsible for Care Act Assessment Safeguarding VARM Mental Capacity Assessments/Best Interests decisions and Court work.

As a Grade 9 Community Support Worker you will be expected to have sufficient experience knowledge and skills to manage your workload under supervision to deliver a high quality person centred accountable statutory social care. This includes demonstrating competency undertaking Care and Support Assessments Mental Capacity Assessments and making Best Interest decisions.

You will require a valid full driving licence or access to a vehicle for this position.

About You

To apply for the CSW post you must:

  • Hold a Level 2 in Maths and English and relevant health/social care qualifications a full Level 2 or 3 qualification in adult social care for example NCFE CACHE Level 2 Diploma in Care or equivalent.
  • Have relevant experience of working with adults in a social care setting and of working with other agencies within a fieldwork environment. And knowledge of the needs and issues of adults and carers including people ethnic minorities disability and age issues
  • Understand the need to undertake need and risk assessments and the issues/needs that adults and their families/carers may have.
  • Have a good understanding of social care organisations and their roles and responsibilities as part of the wider community and voluntary sector.
  • Be a confident communicator who is skilled in negotiation persuasion assertiveness and conflict management and able to build effective working relationships.
  • Be able to recognise the limits of own accountability and responsibility and to seek appropriate support and advice when required.
  • Be able to manage a caseload and work to clear objectives.
