This Nurse Navigator position will work primarily in the Northeast Philadelphia region; primarily at Fox Chase and Jeanes Campuses. The Nurse Navigator functions to provide access to quality care for Temple employees who require Occupational Health collaboration with the multidisciplinary team assists employees along the continuum of care from initial reporting of work injury or request of a service coordination of care & follow ups reporting. The navigator will serve as a clinical resource with expertise in Occupational Health care management while also providing guidance to the employee related to scheduling appointments removing access related barriers including insurance and transportation and answering clinical care related question. Certified Occupational Health Nurse must be obtained within 36 months of hire or transfer.
Education
Bachelors Degree Nursing - Required
Masters Degree in Nursing - Preferred
Experience
5 years in nursing - Required
2 years in a nursing role within Occupational Health - Preferred
Licenses
PA Registered Nurse License - Required
Cert Occupational Health Nurse Within 3 years - Required
