The Community Outreach Specialist I
Educates community members on a variety of health and injury prevention topics based on most recent evidence according to knowledgebase that has been verified through a competency validation process. Formats include: event staffing class presentations and coordinating lay-leader group sessions.
May conduct health screenings according to most recent best practice guidelines under the supervision of a Registered Nurse after completion of relevant education and validated competency by demonstration. Concurrently provides individuals with screening results and appropriate education related to health promotion and risk mitigation.
Mentors Community Health workforce staff on subject matters in which competency has been demonstrated.
Evaluates outreach activities and provides pertinent feedback to leadership regarding outcomes including efficacy and relevance. Provides suggestions for new services and changes in current practice to ensure alignment with departmental and organizational goals.
Collaborates with other Carilion Clinic departments as requested to align initiatives that improve the health of the communities we serve. Provides support for outreach activities sponsored by other departments and consulting with experts to provide timely and relevant evidence-based information to individuals and partners in the community.
Represents Carilion Clinic through positive interactions with the community and media.
Provide timely accurate information to increase awareness of health issues and injury prevention according to demonstrated knowledgebase. Promotes programs and services offered by Community Health & Outreach as well as our organizational and community partners.
Promotes a safe professional team-oriented work environment with a focus on service excellence.
Focuses on improving the health of and preventing injuries in underserved populations.
Maintains goals that include reducing health care disparities and helping communities address barriers found in social determinants of health.
Education: High School Diploma GED or equivalent required.
Experience: 1 years of professional experience required.
Licensure certification and/or registration: Valid Virginia Drivers License.
Other Minimum Qualifications: Must be service-oriented and possess excellent verbal and written communication skills. Requires the ability to educate clients of any age or SES level on health issues; demonstrates knowledge and skills to provide educational components knowledge of principles of growth and life span development assesses specific participant information to identify risk factors. Proficient in Microsoft Office applications highly desirable. Must be capable of working independently with minimal supervision and possess decision-making skills based on expertise and judgement.
