This is a fantastic opportunity to help shape a new service while supporting adults with a range of needs to live safe fulfilling and independent lives.

About the Role

You will provide person-centred support to individuals with learning disabilities autism mental health needs or health conditions across different care settings.

Shift Patterns

This role may includea mix of:

  • Day shifts
  • Late shifts
  • Sleep-in shifts
  • Waking night shifts
  • Every other weekend working

Flexibility is essential to meet the needs of the service.

  • Delivering high-quality support
  • Assisting with personal care daily living tasks and health needs
  • Promoting independence choice and dignity
  • Supporting individuals to access the community and achieve personal goals
  • Following care plans risk assessments and clinical guidance
  • Working collaboratively as part of a new team
