Hospice Registered Nurse (RN) Case Manager

$5000 Sign On-Bonus

Field-Based Full-Time
Eaton County

Your Role as a Hospice RN Case Manager

  • Conduct comprehensive assessments addressing physical psychosocial emotional and spiritual needs

  • Develop implement and coordinate individualized interdisciplinary plans of care

  • Collaborate closely with physicians Clinical Supervisors and the hospice care team to ensure continuity of care

  • Educate patients families and caregivers on hospice philosophy pain management and symptom control

  • Provide compassionate support at time of death and complete bereavement assessments

  • Communicate changes in patient condition and environmental concerns promptly and effectively

  • Serve as a clinical resource and educator for staff in alternate care settings such as nursing homes and assisted living communities

What Were Looking For

  • Hospice experience is highly preferred

  • Graduate of an accredited nursing program (Certificate Diploma Associate or Bachelors degree)

  • Current unrestricted State RN license

  • Minimum of 1 year of clinical nursing experience

  • Ability to travel within the service area (approximately 50%)

  • Ability to lift 50100 lbs and perform hands-on patient care

  • Valid drivers license auto insurance and reliable transportation
