Hospice Registered Nurse (RN) Case Manager
$5000 Sign On-Bonus
Field-Based Full-Time

Your Role as a Hospice RN Case Manager
Conduct comprehensive assessments addressing physical psychosocial emotional and spiritual needs
Develop implement and coordinate individualized interdisciplinary plans of care
Collaborate closely with physicians Clinical Supervisors and the hospice care team to ensure continuity of care
Educate patients families and caregivers on hospice philosophy pain management and symptom control
Provide compassionate support at time of death and complete bereavement assessments
Communicate changes in patient condition and environmental concerns promptly and effectively
Serve as a clinical resource and educator for staff in alternate care settings such as nursing homes and assisted living communities
What Were Looking For
Hospice experience is highly preferred
Graduate of an accredited nursing program (Certificate Diploma Associate or Bachelors degree)
Current unrestricted State RN license
Minimum of 1 year of clinical nursing experience
Ability to travel within the service area (approximately 50%)
Ability to lift 50100 lbs and perform hands-on patient care
Valid drivers license auto insurance and reliable transportation
