Community of Hope is seeking a Community Health Worker to help promote appropriate use of healthcare services while improving access to care and empowering patients to make informed health decisions. As a Community Health Worker you will serve as a vital link between healthcare providers and the community fostering the improvement of health outcomes by providing health promotion education care coordination and connection to resources to individuals and families. This is a full-time position located at our Family Health & Birth Center in NE Washington DC.

What Youll Do

  • Identifies eligible patients for Community Health Worker services through reports in Relevant referrals from COH providers nurses Population Health staff and other members of the care team.
  • Conducts outreach to eligible patients within 2 business days of referral offering CHW services such as brief interventions short term care management (STCM) support emergency room follow-up and CHW-supported group care.
  • Provides brief interventions in clinic and telehealth visits to patients with chronic disease. Ensures patients receive timely and culturally sensitive support and coordination across the interdisciplinary team promoting informed independent patient decision-making.
  • Enrolls eligible patients in STCM services. Conducts initial intake within 1 week of enrollment completing screenings identifying barriers to care and determining what interventions and support the patients desires.
  • Develops a Plan of Care that addresses patient priorities for improving chronic disease self-management improving navigation of the health care system and reducing barriers to care.
  • Builds patient health literacy and facilitates patient empowerment by providing education and resources regarding health conditions medication management healthy lifestyles and chronic disease self-management.
  • Supports healthcare navigation by scheduling appointments making reminder calls arranging transportation following up after missed visits and helping patients with re-engagement with primary care and other services.
  • Connects patients to clinical and community resources for social determinants of health (SDoH) needs by providing tracking and following up on referrals. Maintains an up-to-date broad list of resources.
  • Maintains a panel of patients enrolled in STCM by maintaining ongoing communication with patients at least every 2 weeks through face-to-face encounters telephone video or patient portal messaging to support outreach engagement follow-up graduation and disenrollment.
  • Educates patients on the appropriate utilization of health services to avoid unnecessary utilization of emergency services and provides alternate resources to support health needs when emergency services are not warranted.
Requirements

Must-Haves

  • High school diploma or G.E.D (Associate Degree or higher preferred) or equivalent experience required.
  • Valid BLS Certificate.
  • Prior experience working with community resources and social services in DC.
  • Computer skills and proficiency in the use of handheld mobile device technologies.
  • Ability to understand and apply clinical social and environmental aspects of chronic disease treatment and self-management.
  • Ability to travel between COH sites or relocate to a different COH site on a full or part -time basis.
  • Ability to conduct home visits. Valid drivers license and vehicle required as well as proof of auto insurance.
  • Proven ability to work effectively in team environment.
  • Proof of required vaccinations is required. COH will consider requests for reasonable accommodations for anyone who cannot be vaccinated for a religious or medical reason subject to applicable law.

Nice-to-Haves

  • Two years work experience; prior experience in public health or human services and/or in community health worker or similar role.
  • Excellent interpersonal skills ability to show empathy and ability to interact professionally and gain trust with culturally diverse individuals.
  • Ability to handle confidential information with discretion and professionalism.
