Certifications Required: BLS

Position Overview

We are seeking an experienced RN House Supervisor to provide leadership and clinical oversight during off-shifts (evenings nights weekends holidays) for a 180-bed long-term care facility. The House Supervisor serves as the clinical and operational resource for five specialized units ensuring safe patient care regulatory compliance staff mentorship and effective patient flow across the organization.

This role requires strong charge experience crisis management skills and the ability to lead in a high-responsibility environment.

Facility & Unit Overview

Average Daily Census: 180 Residents

Specialized Units Include:

  • Neurobehavioral Unit Residents with dementia/Alzheimers including behavioral challenges

  • Rehabilitation Unit Post-orthopedic and cardiac procedure patients transitioning home

  • Secure Dementia Unit Residents at risk for elopement

  • Long-Term Care Units Chronic and custodial care residents

Common Diagnoses:

  • Dementia & Alzheimers

  • Diabetes (DM)

  • CVA (Stroke)

  • Cardiac conditions

  • Orthopedic recovery

Special Equipment & Clinical Exposure

  • Wound VACs

  • PICC Lines (care & management)

  • Nephrostomy tubes

  • Tube feedings

  • Indwelling catheters

  • Colostomies

Key Responsibilities

  • Provide clinical leadership and supervision across all facility units during assigned shifts.

  • Serve as a resource for problem-solving and clinical decision-making.

  • Facilitate staff mentoring coaching and performance oversight.

  • Support centralized staffing and productivity standards.

  • Oversee new admissions and patient flow.

  • Manage emergency preparedness code response and escalation processes.

  • Ensure compliance with regulatory standards (DPH TJC OSHA HIPAA).

  • Monitor and report sentinel events and incidents.

  • Handle de-escalation and management of assaultive behaviors.

  • Support documentation processes including death reporting and regulatory compliance.

  • Maintain infection control and isolation protocol standards.

  • Ensure workplace safety and staff fitness-for-duty compliance.

Required Qualifications

  • Active New York State RN License.

  • Minimum 2 years of experience.

  • Previous Charge Nurse experience Required.

  • Experience in Skilled Nursing Facility (SNF) setting.

  • EPIC EMR experience Required.

  • BLS Certification.

  • Strong knowledge of federal and state regulations related to long-term care.

  • Experience supervising nursing staff across multiple units.
