Job Summary
The Wellness Navigator serves as a vital member of the care team supporting patients throughtheir healthcare journey by providing care coordination linkage to services and advocacy forhealth equity. This role works across DAP Health clinics and programs focusing on reducing
barriers to care promoting patient engagement and improving health outcomes for diversepopulations. The Wellness Navigator collaborates with clinical and administrative staff externalpartners and community organizations to ensure continuity of care and access tocomprehensive services.
Supervisory Responsibilities: None
Essential Duties/Responsibilities
- Develop high level expertise in Epic population management to track high-risk patientsand support appropriate engagement in care and utilization of services
- Ensure appropriate and timely documentation of all patient encounters using appropriatetemplates and documentation workflows
- Maintain a caseload of patients tracking attendance identifying barriers and assistingwith appointment scheduling and follow-up
- Provide outreach to patients to support timely initiation and completion of care
- Collaborate with clinical and administrative staff to address barriers to care and facilitatewarm hand-offs to external providers and community organizations
- Coordinate and participate in multidisciplinary case conferences as needed to supportcare planning and continuity
- Ensure that all care documentation is complete up-to-date and available to all careteam members
- Support and collaborate on quality assurance improvement and special projects relatedto patient care
- Maintain and track core performance indicators providing reports as requested
- Assist patients in accessing and utilizing available services within DAP Health andtransitioning to other care as needed
- Participate in events and outreach functions related to assigned program areas
- Understand and promote the organizations commitment to high-quality patient-centeredcare
- Participate in meetings training and collaborative activities as required
- Perform other duties as assigned
QualificationsRequired Skills/Abilities
- Strong interpersonal and communication skills; ability to build trust with diversepopulations
- Ability to explain complex health information in a culturally sensitive and patient-centeredmanner
- Excellent organizational and time management skills with the ability to manage multiplepriorities and follow-up effectively
- Ability to work independently and collaboratively within a multidisciplinary team
- Critical thinking and problem-solving skills to identify patient needs and appropriateinterventions
- Flexibility and adaptability to respond to patient needs and program changes
- Ability to speak in front of a group of professionals and be comfortable
- Effective facilitation skills
- Bilingual (English/Spanish) preferred with sensitivity to linguistic and cultural needs ofpatients
- Proficient in using EHR patient tracking systems and basic MS applications
Education and Experience
- Three years experience in a clinical setting or community health setting
- Three years experience in care coordination
- Knowledge and experience of health system navigation connecting patients to socialservice support agencies and resources
- Knowledge of State programs
- Familiarity with EHR and patient tracking systems
- Current BLS certification obtained through the American Heart Association or AmericanRed Cross
Working Conditions/Physical Requirements
- This position is on-site at a DAP Health Clinic
- Requires current and valid drivers license and current personal auto insurance
- Able to travel to DAP Health locations throughout San Diego and Riverside
- This job operates in an office and clinical settings and requires frequent times of sittingstanding repetitive motion and talking.
- Ability to lift up to 24 pounds
