Job Description
Responsibilities:
Provide comprehensive medical care to patients aged 6 months and older in ambulatory care settings.
Conduct consultations by examining patients reviewing medical records and reports researching information and developing care recommendations.
Adhere to professional standards practice policies federal state and local requirements and regulatory guidelines.
Collaborate with other providers to develop diagnoses and treatment options.
Prescribe or administer treatment therapy medication vaccinations and other specialized care to prevent or treat illness disease or injury.
Order perform and interpret tests; analyze records and examination information to diagnose patient conditions.
Record and maintain patient information including medical history reports and examination results.
Monitor patients conditions and progress adjusting treatments as necessary.
Review health risk assessments (HRAs) to help determine care plans.
Explain procedures test results and prescribed treatments to patients.
Refer patients to specialists or other practitioners when appropriate.
Participate in implementing a multi-disciplinary patient-centered medical home approach.
Stay current by engaging in continuing medical education reading professional publications maintaining networks and participating in professional organizations.
Perform other duties as assigned.
Qualifications:
Graduate of an accredited medical school and licensed to practice in New York State.
Board certified or board eligible.
Minimum of 1 2 years of experience preferred.
Ability to travel to other sites for coverage as needed.
Bilingual in English and Spanish or Creole/French preferred.
