UnityPoint Health Fort Dodge provides coordinated clinic hospital and home-based care for patients in Fort Dodge and surrounding communities. As a designated regional referral center Trinity Regional Medical Center offers a wide range of primary and secondary care services. Trinity is accredited by DNV Healthcare Inc. and has achieved Pathway to Excellence designation by the American Nurses Credentialing Center.

General

  • Established team of 1 Medical Director and 4 Advanced Practitioners
  • Established practice serving the community
  • Experienced staff both clinical and non-clinical who know the patients and the community
  • Fully integrated system with clinics and hospitals on EPIC EMR to improve care coordination

Responsibilities

  • Palliative Care services provided in patients homes clinic and/or nursing home setting
  • Average of 8-10 patients per day; no nights or weekends
  • Shared call (telephone only) amongst the providers. Currently 1:17 Triaged through MyNurse
  • Clinic hours: Monday-Friday 8am-5pm no nights weekends or holidays 4 day work week option available.
  • Evaluates interprets and coordinates information to make decisions and recommendations regarding patient care
  • Patient-focused friendly and willing to collaborate in a team environment
  • Excellent communication (written and verbal) analytical and problem-solving skills

Qualifications

  • Nurse Practitioner or Physician Assistant
  • Experience in post-acute and/or long term care facility preferred
  • Palliative Care certification preferred
  • Ability to work as a team member
  • Hold an unrestricted medical license in the state of Iowa
  • Hold current and unrestricted state and federal authority to prescribe and administer medications as necessary or appropriate to the Advanced Practitioners field of practice
