Pay: $2600/week
Completion Bonus: $2500 per contract (up to $7500 after three contracts)
Job Description
Travel Physical Therapist needed for a home health setting providing therapy services to patients in their homes. Schedule is Monday through Friday with approximately 40 hours per week and an average of 5 patient visits per day. Therapist will travel between patient homes and receive mileage reimbursement. Charting systems used include Netsmart and Epic. Candidates must be quick learners self-starters and able to ramp up quickly. Housing in the area can be challenging especially for travelers with pets.
Key Responsibilities
- Provide physical therapy evaluations and treatments in patient homes
- Manage an average caseload of about 5 visits per day
- Develop and implement individualized care plans
- Document visits and care plans in Netsmart and/or Epic
- Coordinate with interdisciplinary care teams
- Travel between patient locations and track mileage
- Maintain compliance with home health and safety standards
Qualifications
- Active Oregon PT license required at time of submission
- BLS certification required
- Home health experience preferred
- Ability to work independently and learn systems quickly
- Strong time management and patient care skills
