The Opportunity

The Team Leader supports hands-on shifts tailored to the service roster and house support addition to offering on-site coaching and support to staff this position will aid in the daily operations of the SIL and promote quality support to participants.

The Team Leader NDIS Services plays an important hands-on role in the planning and development implementation review and evaluation of a persons Individual Person-Centred Plan whilst also providing supervision and development to staff.

Key Deliverables

  • Lead and oversee quality support deliverythat fosters choice independence and achievement of participant goals.
  • Drive Person-Centred Planning processesin collaboration with the team and Service Manager.
  • Monitor and ensure health and wellbeing needsof participants are met with accurate and timely documentation.
  • Act as a consistent point of contactfor participants and their families to discuss supports and address concerns.
  • Identify document and assess client support needsto ensure tailored service delivery.
  • Supervise and support staff modeling professionalism and integrity in all interactions.
  • Maintain effective communication and positive stakeholder relationships both internal and external.
  • Ensure compliance with policies and procedures including WHS and safeguarding standards.
  • Provide hands-on support where required including personal care medication administration and transport.
  • Champion a culture of teamwork and continuous improvement contributing to innovative and quality practices.

The Essentials

  • Tertiary qualification in Social Welfare Social Work Psychology or Disability Services and/or commensurate experience
  • Demonstrated experience in the delivery of services to people with a disability
  • Established coordination and problem-solving skills.
  • Demonstrated knowledge and understanding of NDIA planning processes
  • Ability to think creatively and use initiative to work independently or as part of a multidisciplinary team.
  • Ability to effectively develop and maintain positive working relationships and networks with government and non-government agencies and services.
  • High level interpersonal skills that support positive and productive relationships.
  • Strong written and computer skills for the creation of plans programs reports correspondence and other documents.
