Logan Health Conrad is hiring a PRN Social Services Advocate to help support our residents and their families!

If you are organized people focused and confident navigating complex paperwork and conversations this is a role where you will truly matter. You will serve as a key coordinator between residents families and the systems that support them while working alongside a caring team oriented group that shows up for each other every day.

This PRN (scheduled as needed) position will be scheduled to cover vacations and sick time of our current Social Services Advocate.

Located at 805 Sunset Boulevard in Conrad MT Logan Health Care Center serves as a home to many of the founders and builders of our community. Surrounded by beautiful golden grain fields and in the shadow of the Rocky Mountain front our facility is a place many call home. We offer memory care long term care as well as skilled nursing services. It offers a beautiful clean and bright environment and prides itself on helping residents feel respected cared for and treasured.

Heres some of what youll be doing in a typical week!

  • Serve as liaison between residents families Medicare Medicaid and Social Security

  • Review referrals from hospitals facilities and families for financial and admission eligibility

  • Coordinate admissions facilitate required forms and ensure all paperwork and data entry are accurate and complete

  • Manage Medicaid applications and financial documentation

  • Work closely with the billing department and the interdisciplinary team

  • Coordinate and participate in care conferences and perform behavior care planning

  • Address resident and family concerns complaints and grievances

  • Assist with discharge planning and connect residents to community and financial resources

  • Participate in state surveys and assist with abuse investigation

Who you are:

  • Highly detail oriented and can complete complex financial and legal paperwork with accuracy

  • Comfortable navigating technical processes such as Medicaid paperwork and spend down

  • Organized and able to manage multiple priorities at once

  • Compassionate independent and self directed

  • Calm under pressure and steady in stressful situations

  • Professional when addressing grievances or participating in investigations with a high degree of confidentiality

Experience in social services or healthcare is preferred but not required. Strong administrative experience and comfort with detailed documentation are essential.

Qualifications:

  • Degree from an accredited school of social work human service field preferred. Other combinations of education and work experience may be considered.

  • Bachelors degree in Social Work preferred but not required.

  • Excellent organizational skills detail-oriented a self-starter possess critical thinking skills and be able to set priorities and function as part of a team as well as independently.

  • Excellent verbal and written communication skills including the ability to communicate effectively with various audiences.

  • Excellent interpersonal skills with the ability to manage sensitive and confidential situations with tact professionalism and diplomacy.

  • Commitment to working in a team environment and maintaining confidentiality as needed.

  • Possess and maintain computer skills to include working knowledge of Microsoft Office Suite and ability to learn other software as needed.

Other Job Specific Duties:

  • Bridges the gap between clients and the health/social service systems to assist in navigating and connecting to the resources needed to improve clients quality of life.

  • Facilitates moves and assists clients in obtaining personal needs and transfer and discharge needs/issues as applicable to assigned area(s).

  • Completes psychosocial assessments within timelines and follows the department process of individualized plans of care and documentation of progress toward goals as applicable to assigned area(s).

  • Builds relationships with client by listening identifying barriers problem-solving and assisting client to develop more effective self-management skills.

  • Attends care conference to discuss client needs/issues and addresses concerns in a proactive and professional manner. Provides support and information to improve the quality of life for clients as applicable to the assigned area(s).

  • Assists clients/families with the psychosocial aspects of transitioning to Transitional Care and/or Long-Term Care as applicable to assigned area(s).

  • Proactively utilizes testing/screening tools processes to make referrals to community agencies and assists with completion of applications as appropriate. Develops maintains and utilizes a current list of community resources.

  • Interviews and compiles information to complete social histories for assigned clients.

  • Provides education related to cultural diversity abuse neglect client rights and responsibilities effective problem solving conflict resolution community health and social services available as applicable to assigned area(s).

  • Supports Logan Health Care Coordination team by communicating identified issues related to Social Determinants of Health (SDOH) in a proactive and professional manner as applicable to assigned area(s). Collaborates with appropriate stakeholders for optimal care and services coordination.

  • Assists clients in completing forms and gathering necessary supporting documentation. Follows-up to ensure notices and/or requests for information are responded to in a timely manner. Follows- up with the application agency (Medicaid/Social Security/OPA etc.) and the client through completion and advocates on behalf of a client as applicable for assigned area(s).

  • Encourages and assists with obtaining a primary care provider completion of Advance Directives and/or End of Life documents as applicable for assigned area(s).

Maintains regular and consistent attendance as scheduled by department leadership.

Shift:

Day Shift - Variable Hours (United States of America)

Schedule: PRN scheduled as needed 8-hour day shifts Monday - Friday. No weekends.

Logan Health operates 24 hours per day seven days per week. Schedules are set to accommodate the requirements of the position and the needs of the organization and may be adjusted as needed.

PRN positions (also referred to as Casual As Needed or Per Diem) at Logan Health offer flexibility and are designed to provide coverage during busy periods employee absences and vacations making them an excellent opportunity to support the team as needed. PRN employees can participate in a matching 401(k) plan and have access to the Employee Assistance Program. Additionally they may qualify for hourly differential pay for certain shifts or hours offering extra compensation based on department-specific scheduling needs.Pay rates and scheduling practices may vary by department.
