Job Summary

Navitas Healthcare LLC is seeking an experienced Physical Therapist to provide home care services. The PT will evaluate and treat patients in their homes to improve mobility strength balance and overall independence.

Responsibilities

Perform home-based physical therapy evaluations
Develop and implement individualized treatment plans
Provide therapeutic exercises gait and balance training
Assess home safety and recommend equipment/modifications
Educate patients and caregivers on exercises and fall prevention
Monitor progress and update care plans as needed
Document all visits and treatments in EMR
Collaborate with interdisciplinary healthcare team

Requirements

Minimum 2 years of Physical Therapist experience
Previous home care experience required
Active New Hampshire PT License
BLS (AHA) Certification
Bachelors Degree in Physical Therapy (or higher)
Valid drivers license and reliable transportation
Ability to work independently
