Summary of Job Duties:
The Medication History Technician (Pharmacy Technician IV) is responsible for obtaining and documenting accurate and complete medication histories for patients upon admission transfer or other designated points of care. This role supports medication reconciliation processes by collecting the Best Possible Medication History (BPMH) through patient interviews caregiver discussions and review of multiple information sources. The Medication History Technician (Pharmacy Technician IV) works under the supervision of a licensed pharmacist and in collaboration with the interdisciplinary healthcare team to reduce medication discrepancies and improve patient safety. They will also represent the department by serving on committees and attending meetings as assigned.
Qualifications:
Minimum Qualifications:
HS/GED plus 4 yrs Pharmacy Tech Experience or Bachelors Degree in Pharmaceutical Sciences
1 year to obtain ASHP/ACPE-accredited technician training or certificate program
National Technician Certification (PTCB)
Arkansas Pharmacy Technician Licensure
Preferred Qualifications:
Competent in all areas and technician roles with a focused competency in Medication Reconciliation

Prepares and dispenses medications accurately and efficiently to patients ensuring proper documentation.
Provides support to control drug inventory management by interacting with vendors and distributors processing payments and coordinating deliveries.
Complies with stocking procedures maintains a high level of organization and monitors drug expiration dates.
Performs routine inspections and maintenance to assess compliance with patient care standards.
Prepares action plans to address issues.
Provides training assistance and performance evaluation to pharmacy technicians and interns.
Participates in process improvement projects for safety and emergency preparedness.
Performs other duties as assigned.
