$38.20 - $57.30Mobile Hospital at Home Registered Nurse provides inpatient care to acutely ill patients within their residences as an extension of in-hospital care. Responsible for providing and coordinating comprehensive patient care through the nursing process to deliver safe therapeutic care in accordance with established standards policies and procedures. Using evidence-based practice clinical decision making compassion and skilled communication leads efforts to create the safest patient environment and the best patient experience across the continuum. The Mobile Hospital at Home Registered Nurse provides hospital-level acute care in a patients home within a defined radius around designated hospitals as part of the Advocate Hospital at Home team which may include the following: Physicians Advanced Practice Providers Pharmacists Case Managers Social Workers Physical Therapists and other operational staff according to the Hospital at Home Care Model.
