The Registered Nurse will provide skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. Responsibilities include patient assessments medication administration treatment planning and collaboration with an interdisciplinary team. Strong communication and collaboration skills are essential for effective interaction with patients providers and team members.

Responsibilities:
- Conduct patient assessments.
- Administer medications.
- Develop and implement treatment plans.
- Collaborate with the interdisciplinary team.
- Provide skilled nursing care.
- Perform case management duties.
- Deliver hospice care.
- Maintain accurate patient records.
- Communicate effectively with patients families and healthcare professionals.
Required Skills & Certifications:
- Active Registered Nurse license in the state of California.
- ACLS BLS NIH and PALS certifications.
- Minimum of 2 years of experience in a hospital home health or hospice setting.
- Clinical Assessment Medication Administration and Treatment Planning skills.
- Interdisciplinary Team Collaboration skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Bachelors degree in Nursing or related field.
Preferred Skills & Certifications:
- Not specified.
Special Considerations:
- Must be willing to float between units.
- Must be willing to work every other weekend.
