Carries a caseload as assigned by direct supervisor.
Provides outreach and direct service to consumers in the community in their natural environment to reduce barriers to accessing services.
Intervenes to change attitudes beliefs and behaviors of consumers to improve individual and family level of functioning.
Maintains networking and referral contacts with other human service providers both internally and externally while assessing barriers for successful engagement treatment and discharge.
CompletesPATHenrollment documentation and records accurately in the electronic health record system and internaltracking systems to allow immediate supervisor to document appropriately.
Provides outreach services to engage homeless individuals to link with local resources for shelter entitlements and meeting of basic needs at a minimum of 4 hoursor more per week.
Attends mandatory meetings and relatedPATHconferences or trainings as identified.
Provides case management services including but not limited to: advocacy linkages to community resources housing services.
Provides crisis intervention to stabilize clients condition by utilizing short term crisis intervention techniques and/or referral to other support services.
