Description

In an environment of continuous quality improvement thePharmacist Clinical Coordinatoris responsible fordeveloping implementing and overseeing clinical pharmacy services to ensure safe effective and evidence based medication use. This role serves as a clinical leader and resource to pharmacists technicians nurses and providers while supporting quality improvement regulatory compliance andoptimalpatient the absence of the Pharmacy Directorthe Pharmacist Clinical Coordinatormayberequiredto perform the essential duties of the Pharmacy Director. Exhibits the MHS Standards of Excellence andalways exercises strict confidentiality.

Job Functions:

  1. Lead clinical pharmacy programs with primary accountability for Pharmacy & Therapeutics (P&T) Committee operations pain stewardship and active participation in antimicrobial stewardship activities in collaboration with the Antimicrobial Stewardship Pharmacist.
  2. Assess develop coordinate implement andmaintaindrug policies and clinical pharmacy programs that support safe effective andoptimalpharmaceutical care.
  3. Collaborate with the Director of Pharmacy to develop and implement the Annual Clinical Plan.
  4. Provide leadership in the development review and maintenance of drug policies that promote safe effective and cost-efficient medication use.
  5. Develop implement andmaintainclinically relevant tools that support clinical pharmacy programs andassistwith pharmacy operations; complete required MUEs annually.
  6. Review and evaluate medication orders and patient care plans to ensure compliance with current clinical guidelines includingappropriatedrug selection dosing frequency interactions and routes of administration.
  7. Collaborate with multidisciplinary teams tooptimizepatient outcomes provide evidence-based therapeutic recommendations to providers and remain current with primary literature and national guidelines.
  8. Provide ongoing clinical education to pharmacists technicians nurses and providers.
  9. Participate in medication profile review pharmacokinetic and drug therapy consults renal dose adjustments and patient education as needed;monitordrug therapy laboratory data and drug levels and intervene appropriately.
  10. Assistswith staffingshifts including weekends during times of critical staffing needs
  11. Assumes all other duties and responsibilities as requested by the Director of Pharmacy.


Qualifications

Minimum Education/Experience Required:

  • Current licensure in the State of Ohio as a pharmacist required.
  • 3 years of inpatient Hospital pharmacy experience preferred.

Special Knowledge Skills Training:

  • Verbal and written communication skills are essential manual dexterity and near visual acuity.

Compensation Details:Education experience and tenure may be considered along with internal equity when job offers are extended.
