Dignity Health Medical Foundation is an affiliate of Dignity Health - one of the largest health systems in the nation - with hospitals and care centers in California Arizona and Nevada. Today Dignity Health Medical Foundation works hand-in-hand with physicians and providers throughout California to provide comprehensive health care services to the many communities we serve. Along with our local affiliated Dignity Health hospital (St. Josephs Medical Center) we offer an integrated care delivery system that provides primary care for family medicine and internal medicine cancer care (gynecologic oncology and radiation oncology) endocrinology gastroenterology and general surgery.

Job Summary and Responsibilities
The Navigator RN will provide direct patient care and comprehensive coordination of care for patients and their families/caregivers to eliminate barriers to timely care facilitate flow through system facilitate interactions with clinical care team increase patient and provider satisfaction increase patient quality maintain point of contact with providers and serve as the first point of contact for patients and families.
