We are seeking a full-time 40-hour Clinical Social Worker to join the Marathon team. The entire team supports the following territories:

Enjoy the freedom to design your own schedule while delivering 1:1 care - because your work-life balance matters. The desired schedule is Monday through Friday. Anticipated daily volume is 4-5 visits per day plus chart review visit preparation documentation and care coordination.
Job Summary
This position is responsible for performing social work assessments and interventions for home care with the occasional request to see a home hospital patient. They are dedicated to patient and family centered care that values personal self-determination skilled at managing multiple priorities at once and engaging in creative compassionate and ethical problem-solving. As a member of a multidisciplinary health team they perform comprehensive assessments and develop treatment plans that integrate the medical social and resource issues that impact individual patients families and at-risk populations. They work collaboratively with all healthcare team members to develop and implement treatment plans that support patient-centered plan of care for both individual patients and the medical community.
Essential Functions
1. Provides psychosocial assessments of patients and families with social emotional interpersonal and/or environmental issues. Formulates biopsychosocial assessment disposition and treatment plans.
2. Provides mandated assessments when abuse is suspected and specialized safety assessment when violence is suspected.
3. Collaborate with other professionals to evaluate patients medical or physical condition and to assess client needs as part of the interdisciplinary team.
4. Monitor evaluate and record client progress according to measurable goals described in treatment and care plan.
LICSW Licensure in the state of Massachusetts required
Excellent verbal written and electronic communications skills
Computer proficiency and ability to learn new software
Ability to prioritize work and adapt to changing situations
Travel required within Mass General Brigham Healthcare at Homes geographic area
