- Full time position in pediatric palliative care working in complementary therapy.
- Collaborates with the PPC/HCIB Interdisciplinary team identified patient and family to develop the plan of care that reflects customized therapeutic and psychotherapy goals.
- Serves as a consultant on the PPC and HCIB Interdisciplinary teams regarding therapy and Psychotherapy techniques.
- Provides psychoeducation regarding age-appropriate needs and responses
- Assists the PPC Program Coordinator and Bereavement Coordinator in development implementation and coordination of pediatric bereavement as it relates to therapy practices.
- Participate in PPC/HCIB Interdisciplinary Team Meetings (IDT).
- Provide direct service to hospice or pediatric palliative care patient/families in homes and places they deem home.
- Conduct assessments and provide interventions that meet individual needs.
- Consult with referring team to assess needs and prioritize referrals.
- Maintain and complete required charting records and reports.
- Participate in team meetings as requested.
- Continual program development and evaluation of therapeutic services
- Assist with orientation of new employees and volunteers and continue to educate staff and therapy volunteers.
- Assess therapeutic equipment including inventory maintenance and use of equipment.
- Assist with memorial service upon request.
- Be flexible in meeting patient/family needs that require evenings and weekends.
- Participate in agency committees and functions as requested and appropriate.
- Develops and maintains clinical records for all referred patients documenting patient reactions progress or other outcomes related to care plan goals.
- Completes documentation after each Point of Service in a timely manner that meets the quality of care accuracy and communication standards of the agency.
- Assists patient/family in identifying concerns preparing for difficult events/decisions and seeks resources as necessary to advocate for patient/family needs.
- Coordinates care with patient/family and community providers.
- Assists in the development of program events that promote inclusivity connection and joyfulness.
- Anticipates initiates and responds collaboratively to provide developmentally appropriate therapeutic interventions for patient/family.
- Recognizes addresses and utilizes art therapy to facilitate self-expression connection relaxation mastery and coping.
- Provides diverse art materials appropriate for individual needs objectives and preferred forms of artistic expression.
- Ensures art materials are high-quality age-appropriate and adequately stocked.
- Participates in professional development to keep abreast of the latest developments in the field of art therapy as well as palliative and end of life care.
