Job Summary
University Home Care Inc is seeking compassionate and skilled Registered Nurses (RN) to provide exceptional home care services in Pontiac MI and the surrounding areas. As a Home Care RN you will play a vital role in delivering personalized nursing care to patients in their homes ensuring their health comfort and safety.
Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans.
- Administer medications and treatments as prescribed by physicians.
- Monitor patients health status and communicate any changes to the healthcare team.
- Educate patients and their families on disease management medication use and health maintenance.
- Maintain accurate and up-to-date patient records and documentation.
- Coordinate with interdisciplinary healthcare professionals to ensure continuity of care.
- Provide support and guidance to patients and their families to promote independence and quality of life.
Requirements
- Valid Registered Nurse (RN) license in the state of Michigan.
- Current CPR and BLS certification.
- Minimum of 1 year nursing experience preferably in home care or related field.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Reliable transportation and ability to travel within Pontiac and surrounding areas.
- Ability to work independently with strong organizational skills.
